Auteur : Microbial Drug Resistance
Genre : Forme et santé
Introduction Staphylococcus aureus is the most common bacterial pathogen in humans, causing a wide range of infectious diseases. Since the 1960s, methicillin-resistant Staphylococcus aureus (MRSA) has been recognized as a life-threatening multidrug-resistant bacterium worldwide and has been a major cause of nosocomial infections; therefore, this MRSA is alternatively called hospital acquired- (or healthcare associated) (HA-) MRSA. (16) It is believed that MRSA is generated from methicillin-susceptible S. aureus (MSSA) by acquisition in its chromosome of staphylococcal cassette chromosome mec (SCCmec), a genetic element containing mecA, which is critical to methicillin resistance. SCCmec elements are diverse in their structural organization and genetic content and have been classified into several types and subtypes. (25) Three types of SCCmec elements (types I, II, and III) are carried m

15 Privilege-Resistant Policies in the Middle East and North Africa - Syed Akhtar Mahmood & Meriem Ait Ali Slimane
Auteur : Syed Akhtar Mahmood & Meriem Ait Ali Slimane
Genre : Droit
Renewing the social contract, one of the pillars of the new World Bank Group strategy for the Middle East and North Africa, requires a new development model built on greater trust; openness, transparency, inclusive and accountable service delivery; and a stronger private sector that can create jobs and opportunities for the youth of the region. Recent analytic work trying to explain weak job creation and insufficient private sector dynamism in the region point to formal and informal barriers to entry and competition. These barriers privilege a few (often unproductive) incumbents who enjoy a competition-edge due to their connections or ability to influence policy making and delivery. Policy recommendations to date in the field of governance for private sector policymaking have been too general and too removed from concrete, actionable policy outcomes. This report proposes -for the fir

16 The New Profile of Drug-Resistant Tuberculosis in Russia: - Steve Olson, Forum on Drug Discovery, Development, and Translation, Board on Health Sciences Policy, Institute of Medicine & Russian Academy of Medical Sciences
Auteur : Steve Olson, Forum on Drug Discovery, Development, and Translation, Board on Health Sciences Policy, Institute of Medicine & Russian Academy of Medical Sciences
Genre : Médecine
An estimated 2 billion people, one third of the global population, are infected with Mycobacterium tuberculosis, the bacterium that causes tuberculosis. Spread through the air, this infectious disease killed 1.7 million in 2009, and is the leading killer of people with HIV. Tuberculosis (TB) is also a disease of poverty--the vast majority of tuberculosis deaths occur in the developing world. Exacerbating the devastation caused by TB is the growing threat of drug-resistant forms of the disease in many parts of the world.
Drug-resistant tuberculosis presents a number of significant challenges in terms of controlling its spread, diagnosing patients quickly and accurately, and using drugs to treat patients effectively. In Russia in recent decades, the rise of these strains of TB, resistant to standard antibiotic treatment, has been exacerbated by the occurrence of social, political, an

Auteur : Steve Olson, Forum on Drug Discovery, Development, and Translation, Board on Health Sciences Policy & Institute of Medicine
Genre : Médecine
An estimated 8.8 million people fell ill with tuberculosis (TB) in 2010 and 1.4 million died from the disease. Although antibiotics to treat TB were developed in the 1950s and are effective against a majority of TB cases, resistance to these antibiotics has emerged over the years, resulting in the growing spread of multidrug-resistant (MDR) TB. Due to challenges in timely and accurate diagnosis of drug-resistant TB, length and tolerability of treatment regimens, and expense of second-line anti-TB drugs, effectively controlling the disease requires complex public health interventions.

The IOM Forum on Drug Discovery, Development, and Translation held three international workshops to gather information from local experts around the world on the threat of drug resistant TB and how the challenges it presents can be met. Workshops were held in South Africa and Russia in 2010. The

Abstract We conducted a study to determine if employment at a children's daycare facility increases an adult's risk of carrying Streptococcus pneumoniae in general and antibiotic-resistant S pneumoniae in particular. From January through March 2003, we obtained nasopharyngeal and oropharyngeal specimens from 63 adult workers at 6 daycare facilities and 65 similarly aged controls; all but 2 controls were nonclinical employees at our tertiary care center. Culture and sensitivity data were obtained from all specimens, and written questionnaires were used to gather information on each daycare worker, control, and daycare center. The vaccination records of children at 5 of the 6 daycare centers were reviewed. Odds ratios with 95% confidence intervals were calculated to compare the rates of colonization with S pneumoniae in the daycare workers and controls. Multinomial logistic regression anal

These children are difficult to manage because of the increased adverse effects of the second-line anti-TB drugs and the prolonged duration of treatment. Furthermore, they are often diagnosed late because the history of a drug-resistant contact was not obtained or responded to. Children mainly have paucibacillary disease, which means that diagnosis of a child, especially with MDR TB, usually points to recent transmission in the community and therefore a failure of

39 Methicillin Resistant Staphylococcus Aureus: Carriage Rates and Characterization of Students in a Texas University (Research AND REPORTS) - Clinical Laboratory Science
Auteur : Clinical Laboratory Science
Genre : Ingénierie
INTRODUCTION Since the introduction and widespread use of antibiotics, resistant strains of bacteria have become a major healthcare problem (1). Isolates of Staphylococcus aureus with resistance to beta-lactam antibiotics were first reported in the United States in 1961, and have since continued to evolve. (1,2) Methicillin resistant Staphylococcus aureus (MRSA) has become one of the major antibiotic-resistant pathogens in recent years, and is capable of causing a wide range of skin and invasive infections including endocarditis, pneumonia, osteomyelitis, gastroenteritis, septic arthritis, deep abscess formation, and rarely, necrotizing fasciitis. (3-8) For the past several decades, MRSA has become a serious problem in patients with exposure to the healthcare system, and is responsible for substantial morbidity and mortality in hospitals around the world. (9) Today, healthcare-associated

Common nosocomial pathogens resistant to multiple antibiotics have increasingly been viewed as a major threat to the treatment of hospital and community acquired infections (1,2). It is mainly due to widespread inappropriate use and over-prescribing of antibiotics in clinical medicine, animal husbandry and veterinary practice (3,4). Accordingly, more stringent infection control measures have been advocated in order to reduce the transmission of infection (5), one such measure is the use of disinfectants of various categories. Use of antiseptic impregnated products, such as dressings and indwelling medical devices has seen tremendous increase in hospitals (6). QACs, are the most prevalent forms of cationic surfactants used today, which are produced by the nucleophilic substitution reaction of tertiary amines by an alkyl halide, benzyl chloride, or similar material (7). Amongst QACs, cetri

Auteur : Nose and Throat Journal Ear
Genre : Forme et santé
Abstract We evaluated the efficacy of a new pharmacokinetically enhanced formulation of amoxicillin/clavulanate (2,000/125 mg) twice daily for the treatment of acute bacterial rhinosinusitis (ABRS) caused by Streptococcus pneumoniae, particularly penicillin-resistant S pneumoniae (PRSP; penicillin minimum inhibitory concentrations [MICs]: [greater than or equal to] 2 [micro]g/ml. A total of 2,482 patients received study medication (safety population). Of these, 2,324 were clinically evaluable (efficacy population), and 1,156 of them had at least one pathogen isolated at screening (bacteriology population). S pneumoniae was isolated from 371 patients in the bacteriology population, including 37 with PRSP. Follow-up in the bacteriology population on days 17 through 28 revealed that amoxicillin/clavulanate therapy was successful in 345 of 371 patients with S pneumoniae infection (93.0%)

Epilepsies
48 Drug-Resistant Epilepsies - Philippe Kahane, Anne Berg, Wolfgang Löscher, Doug Nordli & Emilio Perucca
Auteur : Philippe Kahane, Anne Berg, Wolfgang Löscher, Doug Nordli & Emilio Perucca
Genre : Médecine
More than 12 million people with epilepsy have seizures that cannot be controlled by antiepileptic drugs. The term "drug-resistance" is abundantly used in the epilepsy literature but the definitions proposed differ considerably. It is used indifferently in a number of different settings: as a criterion for selection of patients eligible for new antiepileptic drug trials, for the selection of surgical candidates, for the design of epidemiological studies, for the design of studies on quality of life, for the definition of the epileptic encephalopathies (in comparison to more benign epilepsy syndromes), to mention but a few. As a result, available studies are usually not comparable and referral to epilepsy specialists is unacceptably delayed.

The present volume of Progress in Epileptic Disorders includes several focused chapters on all issues relating to drug-resistance and of
