Introduction Staphylococcus aureus is the most common bacterial pathogen in humans, causing a wide range of infectious diseases. Since the 1960s, methicillin-resistant Staphylococcus aureus (MRSA) has been recognized as a life-threatening multidrug-resistant bacterium worldwide and has been a major cause of nosocomial infections; therefore, this MRSA is alternatively called hospital acquired- (or healthcare associated) (HA-) MRSA. Bei Methicillin-resistenten Staphylococcus-aureus-Stämmen handelt es sich um ein Bakterienstamm, welcher ein nationales Problem in den Krankenhäusern darstellt. MRSA-Patienten verursachen höhere Kosten für das Krankenhaus und somit über die Krankenhausfinanzierung (DRG) für die ganze Gesellschaft. Des Weiteren bedürfen MRSA-besiedelte Patienten intensiverer pflegerischer sowie medizinischer Versorgung und bedeuten für das Krankenhaus erheblichen organisatorischen Aufwand. Aus diesen Gründen sollten die Empfehlungen zur Bekämpfung von MRSA, beispielsweise von der KRINKO korrekt umgesetzt werden. Introduction Staphylococcus aureus (SA) is a common etiology for infective endocarditis (IE) in both injectable drug users (IDUs) and non-injectable drug users. It may also occur as a healthcare associated infection (i.e., hemodialysis or line-associated). Worldwide regional variation has recently been described (1). Historically, MRSA has been specifically associated with IDU and IE in Detroit, Michigan although the majority of cases involving the right side of the heart were infected with methicillin-sensitive Staphylococcus aureus (2,3). Huntington, a metropolitan city in WV, appears to frequently receive illegal drugs via illicit trade based in Detroit (4,5). We report an unexpected case of MRSA IE in an IDU living in Huntington and question whether this was related to contact with IDU from Detroit or a reflection of a change in the epidemiology of MRSA.

Abstract Methicillin-resistant Staphylococcus aureus (MRSA) has become an increasingly common cause of difficult-to-treat head and neck infections. We report a retrospective analysis of 3 cases of MRSA otorrhea treated in our clinic between 2007 and 2009. Culture analysis of otorrhea isolates revealed MRSA infections with identical drug sensitivities. Treatment success was achieved using combinations of linezolid with gentamicin ear drops for 3 to 4 weeks or trimethoprim/sulfamethoxazole (TMP/SMX) with gentamicin drops for 6 weeks. This study illustrates the importance of determining individual drug sensitivities for optimal treatment and maintaining current knowledge of the local MRSA strains. Empiric combination therapy of TMP/SMX with gentamicin is an effective first-line treatment for MRSA otorrhea. Regional differences in clindamycin sensitivities warrant clinical discretion. Fluoro
