From 2001 to 2008, the Robert Wood Johnson Foundation funded the Economic Research Initiative on the Uninsured (ERIU), housed at the University of Michigan. Health insurance is widely regarded as essential for financing the production of good health, hut is insurance always benficial for our health? Ex ante moral hazard may induce individuals with insurance to engage in behaviors that they otherwise would not undertake in the absence of insurance. Using data from the 1993 2002 Behavioral Risk Factor Surveillance System, we attempt to isolate the effects of ex ante moral hazard to determine the potential consequence of having health insurance on measures of body weight, in our analyses, we control for a variety of confounding factors that may influence body weight and address the endogenous nature of health insurance. Our results suggest that having insurance is associated with higher body mass but not the probability of being obese. **********

4 Does High Caregiver Stress Predict Nursing Home Entry? - Inquiry

This study estimates how informal care, paid formal care, and caregiver stress or burden relate to nursing home placement. Data came from the 1999 National Long Term Care Survey and were merged with administrative data. Results show that stress is a strong predictor of entry over follow-up periods of up to two years, and physical strain and financial hardship are important predictors of high levels of caregiver stress. The estimates indicate that reducing these stress factors would significantly reduce caregiver stress and, as a result, nursing home entry. We conclude that initiatives to reduce caregiver stress hold promise as a strategy to avoid or defer nursing home entry. **********

This paper measures the extent to which medical groups experience external pay-for-performance incentives based on quality and patient satisfaction and the extent to which these groups pay their primary care and specialist physicians using similar criteria. Over half (52%) of large medical groups received bonus payments from health insurance plans in the period 2006-2007 based on measures of quality and patient satisfaction. This paper examines the "spillover effects" of state laws that mandate inpatient coverage for breast cancer surgery. It looks at outpatient utilization of two types of breast cancer surgery among Medicare fee-for-service patients', who are exempt from state regulation. Using data from the Surveillanee, Epidemiology and End Results cancer registries and Medicare claims, we performed difference-in-differences analyses of patients in nine states from 1993 to 2002. The analyses show that state laws had a significant impact on only the likelihood of outpatient mastectomy, which was reduced by five percentage points. This study looks at a major metropolitan area and examines racial and ethnic differences in the use of high-volume hospitals for 17 services for which there is a documented positive volume-outcome relationship. Using 2002-2008 data from the ongoing Health and Retirement Study, we analyze the determinants of long-term care utilization simultaneously with the determinants of holding LTCI. We find that LTCI has modest effects on the likelihood o fusing long-term care services. For the very frail elderly, private LTCI enhances their access to nursing home care.
